Home Treatment Team

The Home Treatment Team provides a high level of care and support for service users and their carers when there has been a significant deterioration in a person’s mental health and is an alternative to hospital admission.

The service offers:

• Intensive home treatment as an alternative to inpatient admission, until the crisis or immediate care and support needs in the community have reduced.
• Onward referral to other appropriate care and treatment services.
• Collaborative work within community mental health services, with other services; service users; carers and their families.
• An assertive approach to engagement and a flexible response to service user needs.
• A high level of knowledge of local voluntary, health and social care resources.
• Links with inpatient wards to enable appropriate and timely discharges to the service.
• Assessment of people in crisis where Home Treatment Team/admission may be indicated.
• Act as the clinical gatekeeper in core working hours for inpatient admission to adult acute assessment beds.

Referrals are made via Recovery Service North and Recovery Service South; Single Point of Access; or the Liaison Psychiatry Service.

The Home Treatment Team will arrange to visit the service user within 24 hours of a referral being made. During the initial visit, they will develop a care plan in collaboration with the service user and their carer. The care plan will detail the medical and non-medical interventions which are to be provided during the episode of care under the Home Treatment Team, including plans for discharge.

The Team is coordinated citywide and staff are located in geographical teams alongside the Recovery Service North and Recovery Service South.

The service is available seven days a week between 8am and 6pm. (Outside of these hours the SPA / Out of Hours Team are a point of contact for service users or carers who are under the Home Treatment Team to access support).
